For us to provide you the best care possible in an efficient and thorough manner, we request that you come prepared with any and all necessary documents, referrals, up to date insurance information and government issued ID.
We ask that you come prepared to your first appointment with the following information and items:
Please arrive approximately 30 minutes before the start of your first appointment with us. This will allow us plenty of time to get you entered in our system. There may be additional paperwork that needs to be filled out before we get you started.
If you are an existing patient, please arrive 15 minutes before your set appointment. This will give us time to process any co-payment that you have, before seeing the doctor or staff.
FINANCIAL INFORMATION
We accept most insurance coverage, including WORKERS COMPENSATION. Please see a list of health insurance companies we work with below and contact us to discuss your specific needs.
We are out of network with Cigna Connect and Anthem Healthkeepers Plus.
Co-payments (co-pay), co-insurance, deductibles and any previous balances are due at check-in. We accept cash, personal checks, and credit cards. Our staff is available to assist you with any inquiries regarding your insurance company requirements or your account balance. Please call 703-719-8583 if you have any insurance questions or concerns.
Some health insurance plans require us to have a referral from your primary care physician or a pre-certification before we can see you for an evaluation, office visit or procedure. Please help us expedite your treatment by contacting your primary care physician office and have a referral faxed to us at 703-719-4935. Our experienced team works on obtaining authorizations for the procedures. Please be patient during this process as we’re doing everything possible to provide the best service possible.
